End User Agreement

Scout Health Forms helps your pack or troop keep its families' health forms current and in one place. Because these forms hold personal and medical details, we want to be clear about how we handle them - and ask you to agree before you join a unit or upload a form.

Not affiliated with Scouting America. Scout Health Forms is an independent tool. It is not created, endorsed, sponsored by, or affiliated with Scouting America (formerly the Boy Scouts of America) or any local council. "Annual Health & Medical Record" and related form names are referenced only to help you find and track the official forms.

What we store

The health-form files you upload, the family and member names you enter, and the contact email tied to your account. We use this only to track which forms are on file, remind you when one is expiring, and let your unit's leaders confirm coverage for their roster.

Signing in

You sign in through a trusted provider (such as Google) - we never create or store a password for you. We keep only your verified email and a provider identifier.

Who can see your forms

Access is limited to your own family, the leaders of the units your family belongs to, and site administrators. There are no public or shareable links - every download requires being signed in and authorized. We do not sell or share your information, and we don't use it for advertising.

How your data is protected

Your form files are kept in private cloud storage (Microsoft Azure) and encrypted at rest with an encryption key we control - the files can't be read from storage without that key, and we can rotate or revoke it. All traffic to and from the site is encrypted in transit over HTTPS, and the app keeps soft-delete and point-in-time backups so data can be recovered if something goes wrong.

To be clear, this isn't end-to-end encryption: when you or an authorized leader open a form, the app decrypts it to display it. Who can see a form is governed by signing in and the access limits above (your family, your unit's leaders, and site administrators) - never a public link.

Leaving and deleting

You can leave a unit at any time from My Units. When you leave your last unit, your forms are permanently deleted - so download anything you want to keep first. You can also export all of your data, or permanently delete your family and its forms yourself, from the Account & data page - no need to contact anyone.

By accepting, you confirm you've read the above and agree to upload only your own family's forms and to keep them accurate.
